Dental, chiropractic, optometry & vet
Healthcare is the most-regulated counter in commerce. HSA/FSA cards, insurance copays, treatment-plan financing, no-show policies, HIPAA — none of which generic processors handle. Xray sets up healthcare practices with HSA/FSA acceptance that doesn't get rejected, recurring billing for treatment plans, and a card-on-file vault that survives the OCR audit.
Why generic processing fails Healthcare.
HSA/FSA cards getting rejected
The terminal swipes, the card declines, the front desk fumbles. Wrong MCC, no IIAS support, and your patient can't use the benefit they earned.
Treatment plans that fall apart
A $6,400 treatment plan over 8 months — the patient stops paying at month 3. You eat the $4K balance.
Insurance copays at the front desk
Different copay per insurance, different coinsurance per service. The terminal has no idea, the front desk guesses, you over-collect and refund.
Card-on-file that's not HIPAA-safe
Storing card numbers anywhere near patient data is a HIPAA risk you don't need.
What Xray actually configures for Healthcare.
HSA/FSA-eligible item tagging with IIAS
Auto-substantiation per IRS Notice 2007-2 — eligible items tagged correctly, HSA/FSA card runs cleanly without rejection.
Treatment plan recurring billing
Set a plan: $800/month for 8 months on the patient's card. Auto-charge on the day; smart-retry on decline; collection drops to under 4%.
Integrated copay collection
Practice management system pushes the copay to the terminal; cashier confirms, charges, prints — no math, no over-collection.
Tokenized card-on-file vault
Card numbers replaced with tokens; the actual card data never touches your network. HIPAA-aware, PCI-DSS Level 1 compliant.

What it looks like on a Tuesday.
Dental practice on a Tuesday: patient arrives for a $620 crown. Front desk sees copay is $50 via the PMS, terminal auto-loads $50, patient taps HSA card. Eligible item tag means the HSA card processes without rejection. Balance ($570) is billed to insurance. Three weeks later, EOB comes back showing patient owes $180 — system auto-charges the card-on-file (stored at first visit). Patient gets a payment receipt by email. No phone calls, no statements mailed.
Why generic processors fall short.
Generic processors don't configure your MCC correctly, so HSA/FSA cards reject 40-60% of the time. They don't integrate with your PMS for copay flow. And they store card data in ways that put PHI at risk. The wrong setup is a HIPAA breach waiting to happen.
The actual math.
A 3-provider dental practice doing $1.8M with 7,000 visits at flat-rate 2.75% pays roughly $49,500. We move to interchange-plus and HSA/FSA optimization, typically saving $14-20K/year. Treatment-plan retention rises from ~68% to ~93% with smart-retry recurring billing — a $50-80K topline difference.
Healthcare questions, straight answers.
Do you handle HSA/FSA cards correctly?
Yes — proper MCC, IIAS-compliant item tagging, and auto-substantiation per IRS Notice 2007-2. HSA/FSA cards run cleanly through our terminals.
Can I bill insurance through this?
No — we handle the patient-pay portion only. Insurance billing goes through your practice management system (Dentrix, etc.); we sit between PMS and the patient's card.
Is this HIPAA-compliant?
Yes — we tokenize all card data, store no PHI, and sign BAAs with all healthcare clients on request. PCI-DSS Level 1.
What if a patient on a payment plan misses a charge?
Smart-retry runs three additional attempts over 7 days, with SMS notification to the patient. After that the front desk gets an alert to call.
